INVESTIGACIÓN

El Centro de Referencia a nivel global para esta enfermedad está localizado en Estados Unidos, Childrent’s Hospital of Philadelphia (PA).

El equipo está dirigido por la Dra Adeline Vanderver, neuro pediatra cuya trayectoria profesional ha estado vinculada siempre a la infancia y las enfermedades neurodegenerativas.

Desde 2013, cuando se descubre el gen implicado en esta patología, el equipo ha estado trabajando en el modelo de la enfermedad en ratones, estudio de historia natural y modelo en células humanas empleando la última tecnología disponible.

El equipo médico ofrece apoyo y consejo a todos las familias afectadas por la enfermedad, las cuales estamos colaborando de manera activa a nivel global.

Cabe destacar que la financiación de estos proyectos se ha realizado en un alto porcentaje con fondos públicos y también, de manera significativa fondos privados de Fundaciones y familias afectadas por la enfermedad.

Nuestra Fundación pretende impulsar las líneas de investigación que están actualmente en desarrollo mediante la participación en las mismas desde la Unión Europea, concretamente en dos líneas: ASO y edición génica mediante CRISPR.

Medicina genómica

La medicina genómica es el nuevo campo de investigación médica que aprovecha los avances en genética para desarrollar nuevos medicamentos para el tratamiento de enfermedades. Ya es una realidad la existencia de este tipo de medicinas.

Cuando hablamos de terapia génica, el gen relacionado con la enfermedad permanece en el genoma y el tratamiento tiene por objetivo minimizar el efecto de la mutación mediante diferentes técnicas.

Los avances científicos en terapia celular y terapia génica han contribuido al desarrollo con éxito de la edición génica, que está acelerando la aparición de nuevos tratamientos. La tecnología de edición génica actúa a nivel del ADN, reemplazando o reparando el gen mutado.

Ambas terapias, que pueden ser combinadas, están suponiendo el desarrollo de tratamientos revolucionarios que pueden acabar con las enfermedades sin cura en un futuro cercano.

Debemos ser conscientes que las terapias génicas y la edición de genes puede suponer una alteración para toda la vida, por lo que las personas deben ser conscientes de que puede haber efectos a largo plazo (buenos o malos) que no se conocen en este momento.

Edición génica y CRISPR

CRISPR (Clustered Regularly Interspaced Short Palindromic Repeats ) o Repeticiones Palindrómicas Cortas Agrupadas y Regularmente Espaciadas se refiere a una tecnología de edición génica recientemente desarrollada que puede buscar, eliminar y reemplazar secuencias en el ADN de manera muy precisa.

Este proceso, cuya base fue descrita por primera vez por el equipo del Dr. Francis Mojica en 2005, supone una aportación nuclear a la Microbiología y la Biología en general, abriendo grandes perspectivas en el desarrollo de tecnologías de edición génica aplicables a todo tipo de organismos.

CRISPR es una herramienta dinámica y versátil que nos permite llegar a cualquier lugar en el genoma y una tecnología de base por la capacidad para actuar sobre el ADN de cualquier célula, tejido, órgano, sistema.

La herramienta utiliza la combinación de dos moléculas para editar el/los gen/genes relacionados con una enfermedad: una nucleasa (editor) y ARN guía (que indica a la nucleasa el lugar exacto para editar).

La clave de esta tecnología es la ESPECIFICIDAD, la capacidad para editar el ADN en el lugar preciso gracias a la combinación nucleasa – ARN guía.

Dado que la edición génica actúa directamente en el ADN, existen algunos riesgos que podrían alterar el fin terapéutico y provocar efectos secundarios adversos. Estos desafíos se están estudiando actualmente con el objetivo de que las terapias de edición génica de nueva generación sean precisas y seguras. Entre los riesgos más importantes podemos destacar:

  • Modificaciones genéticas no intencionadas: al editar una secuencia de ADN, es posible que se produzcan modificaciones genéticas no intencionadas o reordenamientos cromosómicos, especialmente si se realizan varias ediciones al mismo tiempo.
  • Eventos de edición fuera de objetivo: también existe el riesgo, dependiendo de las secuencias guía, de modificar las ubicaciones de ADN que no son el sitio objetivo deseado.

Esta tecnología puede acabar en futuro con las enfermedades de base genética y va a ser apoyado de manera activa por nuestra Fundación en los próximos años.

La Fundación esta trabajando para seleccionar un equipo de investigación para apoyar el desarrollo de esta terapia aplicado a la Leucodistrofia TUBB4A.

Terapia génica antisentido (ASO)

El diagnóstico de la enfermedad se basa en la sintomatología (retraso en el desarrollo neuromotor), pruebas de imagen mediante resonancia magnética y pruebas genéticas.

La Resonancia Magnética permite determinar las áreas del Sistema Nervioso Central afectadas en el desarrollo de la enfermedad y que están bien definidas.

Los análisis genéticos se realizan en muestra de sangre y determinan la presencia o no de mutaciones en el gen TUBB4A. Es definitivo para tener el diagnóstico final.

La terapia ASO puede ser diseñada para actuar de una u otra forma. Así, es necesario tener un detallado conocimiento del mecanismo de actuación del gen/genes mutados y diseñar los oligonucleótidos para excluir, silenciar, evitar o seleccionar zonas concretas del gen.

Los oligonucleótidos anti sentido requieren sistemas de transporte capaces de liberarlos intracelularmente, donde hacen su función. Algunos pueden provocar una respuesta inmune que debe evitarse.

La mayoría de los oligonucleótidos anti sentido acaban acumulándose en el hígado y en el riñón y debe ser tenido en cuenta el efecto tóxico ya que esta terapia es permanente en la vida del individuo.

La terapia anti sentido es específica de un gen y de un tipo de mutación, lo que requiere el desarrollo y optimización individualizados, con lo que esto implica en la planificación de un ensayo clínico.

En 2019 se publicó la historia del Milasen, tratamiento llamado así en honor de Mila, niña afectada con el síndrome de Batten. El procedimiento fue muy rápido por el avance de la enfermedad y obtener el permiso de las autoridades en EEUU para su uso.

Hay una línea de investigación activa para seleccionar un compuesto ASO que pueda ser eficiente en pacientes con Leucodistrofia TUBB4A y ver las posibilidades de plantear un ensayo clínico en un futuro cercano.
